Note: Forms have been added to all sites, links must be created for your consumer to access them. The example below is one option of where the links can be added. They can be added in other locations as well. Please contact Concepts2Code to request links to be added to your site. Please also let us know if you would like these forms to be accessible to any web user or just logged in users.
Consumer Forms allow consumers to enter important information about their account directly to your system without an agent being involved. In addition to the Contact Us form, the following forms are available:
- Bankruptcy Form
- Attorney Representation Form
These forms exist in your site but links are not created to them by default. You may create links on the portal or in an email. If you have questions about creating links to the new forms, please contact us. We are happy to assist. Below is an example of adding link buttons to the home page once the user is logged in.
If a consumer is logged in at the time that they are submitting a form, the form will be linked to each account on file.
If a user is not logged in, the form will not automatically be linked to any account. Any time a form is submitted, an email notification is sent to a representative of the agency. This distribution list can be configured in the system settings.
All submitted forms are searchable at any time by clicking the Forms link at the top of the screen.
Bankruptcy Form
The Bankruptcy form allows your consumers to submit information related to their bankruptcy. The system will prompt the consumer to enter the type of Bankruptcy and additional required information, as shown below.
Attorney Representation Form
The Attorney Representation Form allows your consumers to submit information related to their Attorney. The system will prompt the consumer for the information shown below:

Email Notification & Form Submissions
Once a form has been submitted, the email is sent to your team. You may configure the email distribution list for form submissions. To view the form. Click on the View Form Submission link in the email.
